ZeOmega’s Jiva population health management solution to support care coordination as BJC HealthCare expands accountable care capabilities
FRISCO, Texas – February 18, 2014 – ZeOmega Inc., the premier provider of software solutions for population healthcare management, today announced that BJC HealthCare has selected ZeOmega’s Jiva™ population healthcare management software to enhance coordination and improve patient engagement for more than 20,000 patients in the BJC Accountable Care Organization (ACO). Based in St. Louis, Mo., BJC HealthCare is one of the largest nonprofit health care organizations in the country.
ZeOmega’s Disease Management (DM) solution components, including care analytics, care plans, and configurable assessments, will be integrated into the population health initiative for the BJC Accountable Care Organization, as well as BJC’s self-insured employee membership, and throughout the organization.
With growing population health management needs, including its employees and dependents, BJC was seeking a solution that would enhance management of patient care transitions. Jiva also will provide the ability to help identify BJC ACO patients requiring intervention by care managers or other clinicians in order to develop highly personalized care plans.
In addition to its care coordination capabilities, Jiva will enable BJC to integrate disparate data into one central location, and enable customization and distribution of correspondence based on user specification and preference for the post discharge process.
