The Living With Diabetes Self-Management Education and Support Program is offered at Einstein Medical Center Philadelphia to provide education and the tools to help those living with diabetes improve their quality of life.
The Program is a Diabetes Education Accredited Program by the American Association of Diabetes Educators in recognition of the program meeting the National Standards of Diabetes Self-Management and Support. The accreditation signifies that the program offers a high level of quality and service to the community to meet the needs of those affected by diabetes. Participants work with a diabetes care and education specialist to receive individualized care to help them reach their health goals.
- The program includes ten hours of classroom instruction.
- All instructors are Certified Diabetes Educators (CDE) or CDE-eligible.
- A physician referral to the program is required and is covered by most health insurances.
- Participants are not required to be Einstein patients.
- The instructors work with people who have diabetes along with family members and with their healthcare provider to develop safe, effective, and personalized self-management plans.
“Understanding how to manage diabetes can be challenging and people may get conflicting advice from family and friends,” says Patricia Adams, RN, CDE, Diabetes Care & Education Specialist at Einstein Medical Center Philadelphia. “When participants come to our program, they gain accurate and practical information.”

- Monitoring blood sugar
- Healthy eating
- Risk reduction
- Keeping active
- Stress management
- Problem-solving
- Taking medication
